There are two steps you must complete In order to be included in our Flight School / Instructor Database:

  1. You must complete the form below. Please note that bold fields are required.

  2. If you are a newly-minted CFI, DPE, or SFIE, please fax a copy of your certificate to (516)706-3454, Attn: FlySportUSA. This is because it typically takes several weeks to more than a month before the FAA Web site includes newly issued certificates, and for legal reasons, we can't list you until we verify that your certificate is valid.
